    • The Restorative nurse shall assure that each resident’s need for restorative services is assessed and shall assure the provision of necessary interventions based on the assessed need.
    • The Restorative Nurse assures quality care, residents’ rights, dignity, abuse prohibition and customer service is focused on daily.
    • Develops and encourages a teamwork approach within the restorative department and facility.

    Responsibilities

    • Completes Restorative Nursing Program Evaluation in Vision on non-skilled admissions, re-admissions, change of conditions, or residents transitioning from skilled therapy services within 72 hours.
    • Develops restorative programming to meet residents’ functional needs.
    • Complete restorative and other care plan areas such as ADL’s, bowel and bladder for those on restorative caseload.
    • Schedules restorative programs in Vision for documentation.
    • Completes Quarterly Restorative Program Evaluation form and Restorative Nursing Program Progress Note.
    • Supervise the R.N.A.’s to assure completion of restorative programming.
    • Complete Sections G, H, O, P of the MDS for OBRA assessments on non-skilled residents.
    • Assists with MDS completion as assigned by supervisor.
    • Completes TARP training of direct care staff.
    • Participate in CMI process to assure capturing of care provided.
    • Reviews documentation to identify potential improper coding of minutes with MDS completion.
    • Re-educates C.N.A. staff on proper coding of restorative minutes.
    • Participates in Clinical Grand Rounds with therapy and IDT.
    • Understands Facility Quality Measures and participates in quality assurance programs to improve outcomes.
    • Starts bladder diary on flagged residents, reviews completed three day bladder diaries, and develops scheduled toileting or bladder re-training programs based on completed assessment.
    • All other duties as assigned.

    Qualifications

    • RN/LPN in the State of Illinois.
    • Restorative Certified or must obtain within three months of hire
    • CMAC Certification within three months of hire.
    • Knowledge and understanding of the Nursing Process,
    • Ability to work as a team member.
    • Knowledge and understanding of the RAI process
    • Knowledge of nursing policies and procedures.
    • Excellent written and verbal communication skills.

    • Must be able to read, speak and understand the English language. Physical Requirements:
    • Must be able to propel a resident in a wheelchair
    • Must be able to move items at least 20 pounds
    • Must be able to assist residents in evacuating the building in an emergency
    • Must be able to stand for long periods of time
