    This position is part-time (8 or 12 hours shifts) at the Lexington Elmhurst Square Independent Living facility.


    • The skilled nurse is responsible for the provision of direct care and the coordination of services for the client in their home within the independent living community. The nurse will work with the Physician, the Clinical Supervisor, the resident and his/her family and/or the resident's primary caregiver to develop and support an established plan of care.


    • Illinois State License (RN) in good standing with the State Professional Regulations Board.
    • A minimum of one (1) year clinical experience and skills appropriate for client needs.
    • Current CPR certification.
    • Must be able to read, write, speak and understand the English language (bilingual is a plus).
    • Maintain required CEU’s.

    Position Responsibilities:

    • Provides general resident oversight and skilled nursing care to residents and families in the community setting, following all physician orders, agency policies and procedures and state and federal guidelines.
    • Collaborates with physicians, other appropriate health care professionals, the resident, and his/her designee to develop individualized residents care plans.
    • Ensures a safe environment through ongoing evaluation and reports all concerns for the client’s or nurse’s safety to the appropriate Supervisor immediately.
    • Reports changes in resident status appropriately to other skilled staff, clinical supervisor, primary caregiver and/or MD.
    • Identifies needs for additional healthcare related services and make referrals appropriately.
    • Provides healthcare information and education to residents, caregivers and other agency personnel.
    • Supervises and precepts other Lexington Healthcare staff as directed by Clinical Supervisor.
    • Clearly and accurately completes and submits all clinical documentation in accordance with professional standards of practice, federal and state regulations, as well as the agency and accrediting body standards.
    • Maintains a clean, safe, and orderly work environment and ensures the availability of supplies and properly functioning equipment by following the supply and equipment routine established in each facility.
    • Submits appropriate documentation in time frames specified by agency policy.
    • Complies with scheduling guidelines, maintains flexible availability in order to meet resident needs, and to be available to work one (1) weekend per month, as well as rotation of holidays.
    • Attends continuing education activities and maintains documentation of these activities as required by state regulation for licensure and agency policy.
    • Attends and participates in scheduled staff meetings.
    • Demonstrates personal commitment to the Mission Statement of LEXINGTON by providing excellent healthcare and service that is professional, friendly, courteous and timely.
    • Participates in Continuous Quality Improvement (CQI) activities and other LEXINGTON committees as requested.
    • Assists other team members when necessary.
    • All other duties as assigned.


    Job Requirements:

    • Current Registered Nurse (RN) Illinois State License
    • A minimum of one (1) year clinical experience
    • Home health experience preferred
    • Computer skills adequate for data entry of clinical documentation
    • Current CPR certification
    • Current Illinois Driver’s license and proof of current auto insurance
    • Must be able to read, speak and understand the English language
