    The Intake Specialist is a key partner with other staff of the customer call center, our facilities, and our businesses, their marketing staff, and all referring agencies or consumers. Communication with these business partners is critical, especially with follow-up and a customer service mindset, in order to secure residents / patients for our skilled nursing facilities and home care businesses. Several positions and shifts are available.

    Responsibilities

    • Make independent decision based on business processes to qualify customers for LHN services based on business specification in the SNF, Home Care, and Sleep businesses, clinically and financially.
    • Track and manage all referrals in the LHN customer relationship management (CRM) system.
    • Obtain all necessary information on the needs of the customer, identify services LHN can fulfill for the customer, and secure admission to an LHN facility as warranted.
    • Finalize acceptance with required information for admissions or re-admissions.
    • Communicate with referral partners, payer partners, LHN facilities, LHC home care businesses, and their marketing partners in an expeditious manner on acceptance or denial of an admission.
    • Verify insurance, screenings, qualifying stays and days for admission.
    • Communicate with facility staff to ensure accuracy of information relating to bed availability, planned discharges, resident / patient needs, isolation and/or transfers to hospitals.
    • Develop positive partnerships with referral providers, LHN facility, Home Care businesses, as well at their marketing personnel.
    • Understand and aid business partners with their key performance indicators and key services relating to LHN admission and re-admission processes.
    • Proactively participate in continuous improvement activities for the LHN Intake Center.
    • Team members will be hired for various shifts, share on-call duties, and coverage requirements.
    • Performs other duties as assigned.

    Qualifications

    • Associate’s or Bachelor’s degree preferred or equivalent work experience.
    • Individuals with experience in acute or post-acute settings, admissions work (long-term care, rehabilitation, acute, post-acute), and/or work within call centers are preferred.
    • Flexibility to work occasional evenings and/or limited weekends in exchange for time off during the week.
    • Team members will have to cover some off-hour schedules to cover other employees.
    • The ability to work from home, via internet, occasionally will be required.
    • Passion to create a customer-centric experience for the LHN Intake Center of Excellence.
    • Possess excellent written and verbal communication skills.
    • Basic proficiency in MS Word, Excel, and Internet tools.
    • Must be willing to work and win as a team.
