    This After Hours/On Call RN for Hospice supports the team by being available from Monday at 5:00PM to 8:00AM Monday, Tuesday, Wednesday and Thursday into Friday at 8:00AM.  This is a full time, salaried position. 



    Position Responsibilities:

    • Admissions, evaluations, scheduled visits and emergent visits.
    • Coordinates with the IDT the delivery, coordination and monitoring of appropriate services for patients, families/caregivers.
    • Participates as a member of the interdisciplinary team in the development,implementation, and oversight of the patient care plan.


    • Current professional nursing license to practice in the state Illinois.
    • Current CPR card
    • Current TB
    • Current Illinois Driver’s License and proof of current auto insurance
    • One to three years’ experience in medical, oncology or hospice nursing
    • Excellent assessment and teaching skills
    • Able to make home visits year round, and able to cover a large geographic territory
    • Must be able to read, speak and understand the English language
