    • The Post-Acute Unit Manager is responsible for ensuring the patients are cared for within the Post-Acute areas of the facility, and has full accountability for the clinical outcomes associated with delivering this care.
    • The Post-Acute Unit Manager participates in the assessment, planning, implementation and evaluation aspects of care provided for each patient and collaborates with other members of the interdisciplinary team in providing quality care.
    • The Post-Acute Unit Manager ensures that desired outcomes have been achieved through various monitoring systems.


    • RN in the State of Illinois.
    • Recent experience with acute, post-acute, and rehab. Care.
    • Knowledge and understanding of the Nursing Process of assessment, planning, implementation, and evaluation.
    • Training in rehab. nursing and the rehabilitation process preferred.
    • Excellent written and verbal skills.


    Desirable Skills:

    • Supervision and leadership, critical thinking, time management, communication, and the ability to develop interpersonal relationships with patients and their families.
    • Must be able to read, speak and understand the English language.
