    We are seeking a Information Coordinator for our Hospice Division.  

    • The Information Coordinator is responsible for all processes to maintain medical records, data entry, and administrative tasks as directed by manager/supervisor.
    • They will communicate with clients, families, supervisor, staffing and Lexington SNF building staff to obtain and process referral information.


    • High School Diploma or GED.
    • Data entry training demonstrating strong Computer skills & word proficiency.
    • Strong organizational and verbal skills.
    • Ability to multitask, take direction, and function as a team member.
    • Must be able to read, write, speak and understand the English language.
